How to see your parent’s Wellstar MyChart account with your own login, how to get copies from the records office, and how to keep the results in one place.

See your parent’s Wellstar MyChart account
Hospitals call this proxy access, or family access. You sign in with your own login and switch to your parent’s account, so you never need their password. At Wellstar: the patient signs an adult proxy form.
- 1Print the Wellstar MyChart Adult Proxy and Authorization for Release of Medical Information form.
- 2You fill in page 1. Your parent fills in and signs page 2.
- 3Email it to mycharthelp@wellstar.org or fax it to 770-999-2306.
- 4You see your parent's record through your own MyChart account. The form sets one up if you don't have one.
- 5For help, call the MyChart Help Desk at 470-644-0419.
Get copies without the portal
From the HIM Release of Information.
- 1Download the Authorization for Release Form. A Spanish version is on the page.
- 2Your parent fills it in, signs it, and names you to receive the records. If your parent was seen at several Wellstar sites, send one form and mark all of them.
- 3Your parent faxes it with a copy of photo ID to 770-810-4193, or it is mailed to ATTN: HIM Release of Information at a Wellstar hospital, for example Wellstar Kennestone Regional Medical Center, 677 Church Street NE, Marietta, GA 30060. Wellstar accepts faxes only from patients and healthcare providers.

- Family members:
- For a patient 18 or older who cannot sign because of physical or mental illness, a Durable Healthcare Power of Attorney (or other court appointed documentation) must be provided.
- Cost, as the hospital states it:
- Records delivered to the patient via email, mail, fax, or in-person: $6.50 plus tax. Records delivered to an insurance company, attorney, or third party: the applicable fee will be charged to the requester.
- Time, as the hospital states it:
- Requests for copies will be processed within 7 - 10 business days; however they may take up to 30 days.

Your parent’s rights
- Your parent has a legal right to copies of their own records.
- When your parent asks, the hospital must answer within 30 days. It can take 30 more days once, if it explains why in writing.
- It can charge only a reasonable fee, based on what the copy costs to make.
- These rules also cover an electronic copy your parent asks, in writing, to have sent to you.

How do I keep my parent's Wellstar results in one place?
