See your parent’s MyWellSpan account
Hospitals call this proxy access, or family access. You sign in with your own login and switch to your parent’s account, so you never need their password. At WellSpan: the patient sends an invitation, or you complete a caregiver request form.
- 1Invite route: your parent goes to Your Menu, Sharing, Manage Family Access (Proxy), then clicks Invite Friends or Family.
- 2Your parent needs your email address, and you need their date of birth to accept.
- 3Caregiver route: complete the caregiver proxy request form.
- 4For that route, a legal document must be on file, or an adult proxy agreement is completed in the office.
- 5Proxy requests can take up to three business days.
Get copies without the portal
From the Health Information Management – Release of Information.
- 1Download the Authorization to Use or Disclose Health Information form. It comes in several languages.
- 2Your parent fills it in, signs it, and names you to receive the records. List every WellSpan location on one form.
- 3Mail, email or fax it as the instructions at the bottom of the form say. There is also an online request form; have photo ID ready.
- Family members:
- To request medical records, you must be the patient, or parent, or legal guardian (proof of guardianship is required).
- Cost, as the hospital states it:
- Records sent directly to another healthcare provider – no charge. Records delivered to the patient via email or fax – $6.50 plus tax; via mail – $6.50 plus shipping, handling, and tax. Third party: the fee allowable by applicable state.
- Time, as the hospital states it:
- Average turnaround time is 15 calendar days, however it may take up to 30 days to ship.

Your parent’s rights
- Your parent has a legal right to copies of their own records.
- When your parent asks, the hospital must answer within 30 days. It can take 30 more days once, if it explains why in writing.
- It can charge only a reasonable fee, based on what the copy costs to make.
- These rules also cover an electronic copy your parent asks, in writing, to have sent to you.
Keep the results in one place
Vita Health Coach is an iPhone app that keeps each family member's results in their own file and explains each new result in plain words. Once you have access, add your parent, tap Connect a hospital and search for WellSpan. Paper copies work too: take a photo of each page.
