See your parent’s Virtua MyChart account
Hospitals call this proxy access, or family access. You sign in with your own login and switch to your parent’s account, so you never need their password. At Virtua: the patient submits a request form.
- 1Your parent signs in to Virtua MyChart and opens the Menu next to the Home icon.
- 2They scroll to Sharing and select the adult proxy request form. A PDF version is also on Virtua's page.
- 3They enter your name, date of birth, address, email and phone, then submit.
- 4After it is processed, you get an activation code and sign-up instructions.
Get copies without the portal
From the Health Information Management Department.
- 1Fill in the Release of Patient Information form. Your parent signs it and names you.
- 2Add a note with the date, or about the date, of the visit, procedure or hospital stay.
- 3Mail or fax it to the Health Information Management Department of the hospital where your parent was treated. Each hospital's fax is on the page (for example Voorhees: 856-247-3758).
- 4Or bring the request, the form and a government photo ID in person, Monday to Friday, 8:30 am to 5 pm. Requests by phone are not accepted.

Your parent’s rights
- Your parent has a legal right to copies of their own records.
- When your parent asks, the hospital must answer within 30 days. It can take 30 more days once, if it explains why in writing.
- It can charge only a reasonable fee, based on what the copy costs to make.
- These rules also cover an electronic copy your parent asks, in writing, to have sent to you.
Keep the results in one place
Vita Health Coach is an iPhone app that keeps each family member's results in their own file and explains each new result in plain words. Once you have access, add your parent, tap Connect a hospital and search for Virtua. Paper copies work too: take a photo of each page.
