See your parent’s My Health at Vanderbilt account
Hospitals call this proxy access, or family access. You sign in with your own login and switch to your parent’s account, so you never need their password. At Vanderbilt: you apply in person with legal papers if your parent has diminished capacity.
- 1This route is for a parent 18 or older with diminished capacity. Download the form Account Access for Adults 18 and Over with Diminished Capacity.
- 2Bring the form, your government-issued photo ID and legal papers showing conservatorship or health care power of attorney to your parent's next appointment, the Vanderbilt Health Walk-in Clinic, or the Family Resource Center at Monroe Carell Jr. Children's Hospital.
- 3A Vanderbilt staff member must sign the application. You can fax the legal papers to (615) 875-2820 or email them to MHAVLegal@vumc.org, but not the application itself.
- 4Vanderbilt reviews it. The review can take several business days, and staff will contact you if they need more.
- 5If your parent can still decide for themselves, call My Health at Vanderbilt Help at (615) 421-6428 to ask how they can share access with you.
Get copies without the portal
From the Vanderbilt Health Medical Records.
- 1Your parent can request records online with the Patient Medical Records Request link on the page, or in My Health at Vanderbilt.
- 2Or download the Authorization for Release of Medical Information form. Your parent fills it in, names you with your full address and phone number, and signs it.
- 3Fax it to the facility where your parent was treated. For VUMC: fax (615) 410-2406, phone (615) 322-2062. Other hospitals' numbers are on the page.
- Family members:
- Release of records requires patient authorization. To request another adult's records yourself, you must provide papers such as power of attorney or guardianship.
- Cost, as the hospital states it:
- there may be a small fee for some of these services

Your parent’s rights
- Your parent has a legal right to copies of their own records.
- When your parent asks, the hospital must answer within 30 days. It can take 30 more days once, if it explains why in writing.
- It can charge only a reasonable fee, based on what the copy costs to make.
- These rules also cover an electronic copy your parent asks, in writing, to have sent to you.
