Your parent’s results and records at UT Southwestern
How to see your parent’s MyChart account with your own login, how to get copies from the records office, and how to keep the results in one place.
From UT Southwestern Medical Center’s own pages, checked on November 3, 2026. Steps can change, so follow the official links.
See your parent’s MyChart account
Hospitals call this proxy access, or family access. You sign in with your own login and switch to your parent’s account, so you never need their password. At UT Southwestern: the patient sends the invitation; a paper form is also available.
- 1Your parent logs in, selects Menu, then Proxy Invite under the Sharing header.
- 2They select Invite Someone, fill in your details, agree to the terms and select Send Invite.
- 3You get an email and accept. If you have no account, create one first and return to the email.
- 4Enter your parent's date of birth to verify and get access.
- 5Paper route: complete, sign and return the Request for MyChart Proxy Access form to the clinic. It is processed within 3 business days if nothing else is needed.
Get copies without the portal
From the Health Information Management (HIM) – Release of Information.
- 1Download the Authorization to Disclose Protected Health Information form. A Spanish form and filling-in instructions are on the page.
- 2Your parent fills in every part, signs it, and names you to receive the records.
- 3Send it by fax (214-645-9141), email (Medical.Records@UTSouthwestern.edu) or mail to Health Information Management – ROI, as the page lists.
- 4Ask for electronic copies (PDF). The page says requests are processed electronically unless you ask for another format.
- Family members:
- If the person signing is the patient's legal guardian, executor of the estate, or medical power of attorney, they must send a copy of the legal document showing their authority. The person picking up records must show acceptable ID.
- Cost, as the hospital states it:
- There is usually no charge for records requested by patients. A reasonable fee may apply depending on the volume of records or delivery method. For all other requests, a reasonable fee may apply according to Texas State guidelines.
- Time, as the hospital states it:
- Urgent requests can be processed on the same day (if records are available, complete, and signed and the encounter is closed) by noting "stat/urgent" on the request.

Your parent’s rights
- Your parent has a legal right to copies of their own records.
- When your parent asks, the hospital must answer within 30 days. It can take 30 more days once, if it explains why in writing.
- It can charge only a reasonable fee, based on what the copy costs to make.
- These rules also cover an electronic copy your parent asks, in writing, to have sent to you.
Keep the results in one place
Vita Health Coach is an iPhone app that keeps each family member's results in their own file and explains each new result in plain words. Once you have access, add your parent, tap Connect a hospital and search for UT Southwestern. Paper copies work too: take a photo of each page.
