See your parent’s UR Medicine MyChart account

Hospitals call this proxy access, or family access. You sign in with your own login and switch to your parent’s account, so you never need their password. At UR Medicine: the patient gives permission in the portal or on a paper form.

  1. 1If your parent has MyChart, they log in, open Settings, then Family & Friends, and fill out the form there.
  2. 2They choose Full Access or Full Access (Without Visit Notes).
  3. 3If your parent has no account, they use the Online Access Request Form first, or the paper form MyChart Proxy Authorization: 18 and Over.
  4. 4The paper form goes to the doctor's office, or is mailed or faxed using the details on the form.
  5. 5Online requests get a reply within the week up to 14 business days. Allow at least two weeks for paper.
  6. 6If your parent has diminished capacity, an authorized representative must ask through their UR Medicine doctor's office, not online.
