How to see your parent’s My UNC Chart account with your own login, how to get copies from the records office, and how to keep the results in one place.

See your parent’s My UNC Chart account

Hospitals call this proxy access, or family access. You sign in with your own login and switch to your parent’s account, so you never need their password. At UNC Health: the patient sends the invitation from their own account.

  1. 1Your parent goes to Menu, then Sharing Hub, and selects A family member, close friend, or caretaker.
  2. 2Your parent selects Give ongoing access to your My UNC Chart account, then + Invite someone, enters your name and email, confirms the level of access and clicks Send Invite.
  3. 3You open the email link, log in to My UNC Chart, and enter your parent's date of birth to accept the invitation.
  4. 4Or you ask from your own account: Menu, then Proxy Access Request. Choose Authorization for Another Person to Have Access to My Health Information, or the Health Care Power of Attorney or Legal Guardian form if you hold those papers.
  5. 5Once access is granted, you open your parent's chart from their name or photo and accept the proxy access disclaimer.

  1. 1Download the Patient Request for Access to Protected Health Information form (HIM# 1409s).
  2. 2Your parent fills it in, names you as the person to receive the records, and signs and dates it. Sensitive records, such as mental health, must be ticked to be included.
  3. 3Fax or mail it to the records office of the UNC Health hospital where your parent was treated. Each office's address and fax is listed on the form.
Family members:
An authorized representative can sign instead of the patient and must explain their authority to act for the patient.
Cost, as the hospital states it:
A reasonable cost-based fee may be charged for copies of records being requested
