See your parent’s My Health Online account

Hospitals call this proxy access, or family access. You sign in with your own login and switch to your parent’s account, so you never need their password. At Sutter: the patient invites you or signs the adult proxy authorization form.

  1. 1Your parent can give you access if you are 18 or older and a family member, or have a legal right to manage their care.
  2. 2Your parent invites you from their own My Health Online account, or fills in and signs the Proxy Access Form (Adults 18+). Use the form if you aren't a Sutter patient.
  3. 3On the form, your parent names you as Proxy Representative and ticks your relationship, such as Adult Child. Legal papers may be asked for.
  4. 4Your parent sends the signed form by fax to (877) 607-6484, by email to MHOSupport@sutterhealth.org, or by mail to Patient Services Contact Center, P.O. Box 255386, ATTN: My Health Online Proxy, Sacramento, CA 95865-5386.
  5. 5The access lets you use My Health Online for your parent. It does not let you make health care decisions, and either of you can end it at any time.
  6. 6Help: 800-4SUTTER, weekdays 7:00 am to 7:00 pm.
