See your parent’s MyHealtheLife account
Hospitals call this proxy access, or family access. You sign in with your own login and switch to your parent’s account, so you never need their password. At Stony Brook: the patient asks staff in person during a visit.
- 1During an inpatient or outpatient visit, your parent tells a staff member they want you as a proxy.
- 2Your parent gives staff your email address.
- 3You receive an email invitation and use it to create your account.
- 4Proxy accounts cannot be set up through self-enrollment.
Get copies without the portal
From the Medical Records (Health Information Management).
- 1Use the authorization form for release to a third party. Your parent fills it in, signs it and names you.
- 2If you are your parent's legal representative, use the representative version of the online request form or paper form on the page instead.
- 3Send the form as the page directs. Call (631) 689-8333 to confirm where to send it.
- Family members:
- Different forms are used if you are the patient or a patient representative. If the patient is incapacitated, a legally responsible personal representative can sign.
- Time, as the hospital states it:
- All requests will be processed within ten (10) business days of receipt of the requests.

Your parent’s rights
- Your parent has a legal right to copies of their own records.
- When your parent asks, the hospital must answer within 30 days. It can take 30 more days once, if it explains why in writing.
- It can charge only a reasonable fee, based on what the copy costs to make.
- These rules also cover an electronic copy your parent asks, in writing, to have sent to you.
Keep the results in one place
Vita Health Coach is an iPhone app that keeps each family member's results in their own file and explains each new result in plain words. Once you have access, add your parent, tap Connect a hospital and search for Stony Brook. Paper copies work too: take a photo of each page.
