Your parent’s results and records at Providence
How to see your parent’s MyChart account with your own login, how to get copies from the records office, and how to keep the results in one place.
From Providence’s own pages, checked on November 4, 2026. Steps can change, so follow the official links.
See your parent’s MyChart account
Hospitals call this proxy access, or family access. You sign in with your own login and switch to your parent’s account, so you never need their password. At Providence: the patient sends the invitation from their own account.
- 1Your parent signs in to MyChart at providence.org/MyChart and selects Share my record from the main menu.
- 2In the Invite friends and family section, your parent selects Invite someone and enters your basic details.
- 3You get an email invitation and follow the link.
- 4You enter your parent's date of birth, then review and accept the privacy policy and the invitation.
Get copies without the portal
From the Providence St. Joseph Health Central ROI (hospital records).
- 1For hospital records, download the Patient Request to Access/Disclose a Designated Record Set form.
- 2Your parent fills it in, lists the hospitals and dates of treatment, names you as the recipient, and signs it.
- 3Mail it to Providence St. Joseph Health, Central ROI, P.O. Box 4950, Portland, OR 97208, or fax it to (855) 234-2493. Phone: (855) 234-2491.
- 4That address is for hospital records only. For other records, choose your parent's state on the Providence medical records page. Each state has its own page.
- Family members:
- For a deceased patient, send a copy of the death certificate, the power of attorney, trust or will if available, and the requester's driver's license with the form.
- Cost, as the hospital states it:
- there may be a fee for production of the medical records

Your parent’s rights
- Your parent has a legal right to copies of their own records.
- When your parent asks, the hospital must answer within 30 days. It can take 30 more days once, if it explains why in writing.
- It can charge only a reasonable fee, based on what the copy costs to make.
- These rules also cover an electronic copy your parent asks, in writing, to have sent to you.
Keep the results in one place
Vita Health Coach is an iPhone app that keeps each family member's results in their own file and explains each new result in plain words. Once you have access, add your parent, tap Connect a hospital and search for Providence. Paper copies work too: take a photo of each page.
