See your parent’s Piedmont MyChart account
Hospitals call this proxy access, or family access. You sign in with your own login and switch to your parent’s account, so you never need their password. At Piedmont Healthcare: the patient sends an invite, or you both sign a form.
- 1If your parent uses MyChart, they log in, open Sharing Hub and click Manage friend and family access to invite you.
- 2Or print Piedmont's MyChart Adult Proxy form. You fill it in, and your parent signs the Adult Proxy for Release of Medical Information form. Both of you must sign.
- 3Return both forms to your parent's Piedmont doctor's office.
- 4Piedmont reviews each request carefully. You see the record through your own MyChart account.
Get copies without the portal
From the Medical Records (Release of Information).
- 1Download the request form. A Spanish form is on the page.
- 2Your parent fills in every field, checks the hospital, signs it, and names you to receive the records. Add the service dates and pick one delivery method.
- 3Send it with a copy of the government ID the page asks for. Email both to recordsrequest@piedmont.org with the hospital name in the subject line, or fax them to the hospital fax number printed on the form (Piedmont Atlanta Hospital: 1-833-605-2063).
- 4Questions: the hospital's records number on the form (Piedmont Atlanta Hospital: 404-605-3280).
- Cost, as the hospital states it:
- There may be fees for provision of the information requested; however, records for treatment purposes may be faxed to the patient's healthcare provider when requested at no charge.
- Time, as the hospital states it:
- Under most circumstances, applicable law permits up to thirty (30) days for record requests to be processed.

Your parent’s rights
- Your parent has a legal right to copies of their own records.
- When your parent asks, the hospital must answer within 30 days. It can take 30 more days once, if it explains why in writing.
- It can charge only a reasonable fee, based on what the copy costs to make.
- These rules also cover an electronic copy your parent asks, in writing, to have sent to you.
