See your parent’s MyChart account
Hospitals call this proxy access, or family access. You sign in with your own login and switch to your parent’s account, so you never need their password. At Ohio State: the patient grants access using a form.
- 1Your parent opens the FAQ link to the form for patients 18 and older to grant another adult proxy access.
- 2They log in to MyChart and are taken to the form.
Get copies without the portal
From the Medical Information Management (MIM).
- 1Download the medical records authorization form.
- 2Your parent fills in every section, signs it, and names you to receive the records.
- 3Return it to the address on the form, or fax it (all other requests: 614-366-9442).
- 4Questions: Main Campus 614-293-8657, East Hospital 614-257-2544.
- Family members:
- If you have healthcare power of attorney for a patient in the hospital and need help with access, call Medical Information Management.

Your parent’s rights
- Your parent has a legal right to copies of their own records.
- When your parent asks, the hospital must answer within 30 days. It can take 30 more days once, if it explains why in writing.
- It can charge only a reasonable fee, based on what the copy costs to make.
- These rules also cover an electronic copy your parent asks, in writing, to have sent to you.

Vita Health Coach is an iPhone app that keeps each family member's results in their own file and explains each new result in plain words. Once you have access, add your parent, tap Connect a hospital and search for Ohio State. Paper copies work too: take a photo of each page.
