See your parent’s MyNM account

Hospitals call this proxy access, or family access. You sign in with your own login and switch to your parent’s account, so you never need their password. At Northwestern: the patient invites you in MyNM, or a proxy form is submitted.

  1. 1If your parent uses MyNM, they invite you with the Share My Record function.
  2. 2If your parent does not use MyNM, complete the MyNM Adult Proxy Request form.
  3. 3The form comes in English, Spanish, Polish and Arabic.
