See your parent’s MyNorthwell account

Hospitals call this proxy access, or family access. You sign in with your own login and switch to your parent’s account, so you never need their password. At Northwell: the patient invites you in the portal or consents in person, or you send an online form.

  1. 1Portal route: your parent opens the Sharing Hub in MyNorthwell, invites you and picks View or Full Access.
  2. 2In-person route: you and your parent both go to a Northwell provider's office, and your parent gives verbal consent to staff.
  3. 3Form route: you fill out Northwell's online Adult Proxy Form. Access takes 3 to 5 business days after processing.
  4. 4If you hold power of attorney, include a valid copy with the online form.

Get copies without the portal

From the Health Information Management (HIM).

  1. 1Print the Authorization form. Your parent fills it in, signs it and names you.
  2. 2Send it to the HIM department at the hospital where your parent was treated, by email, fax, mail or in person. Each hospital's contacts are on the page.
  3. 3If your parent cannot sign, send proof of your authority, such as a health care proxy or power of attorney.
  4. 4For scans and lab tests, ask the place that did them.
Family members:
The patient or their legally appropriate personal representative can access medical records with required documentation. For an incapacitated patient, send documents such as durable or healthcare power of attorney, or a healthcare proxy.
Cost, as the hospital states it:
There is no fee if the records are being released to a doctor or healthcare provider; there is generally a reasonable cost-based fee if the records are being released for a different purpose.
Time, as the hospital states it:
Patient requests will be processed within 10 days from the date received. Patients should allow 3 days from submission before calling for status.

Your parent’s rights

  • Your parent has a legal right to copies of their own records.
  • When your parent asks, the hospital must answer within 30 days. It can take 30 more days once, if it explains why in writing.
  • It can charge only a reasonable fee, based on what the copy costs to make.
  • These rules also cover an electronic copy your parent asks, in writing, to have sent to you.
