See your parent’s Connect account
Hospitals call this proxy access, or family access. You sign in with your own login and switch to your parent’s account, so you never need their password. At NewYork-Presbyterian: the patient completes an authorization form at the provider's office.
- 1Your parent completes the Connect Patient Portal Proxy Access Authorization form at their provider's office.
- 2The hospital processes the form.
- 3You get an email to set up a Connect account, or to add your parent's access to the account you already have.
- 4You do not need to be a patient yourself.
- 5If your parent cannot sign because of limited capacity, contact their provider.
Get copies without the portal
From the Medical Records.
- 1Fill in the authorization form, on paper or with the online request form on the page. Your parent signs it and names you.

- 3The online form has a choice for 'I am a Patient Representative' if you are your parent's representative.
- 4Questions: call Customer Service at 646-697-4764.
- Family members:
- The online authorization form has an option for 'I am a Patient Representative'. The page says nothing more about family members.
- Cost, as the hospital states it:
- There is no fee when records are released to your patient portal. Fees may apply for any other release method.

Your parent’s rights
- Your parent has a legal right to copies of their own records.
- When your parent asks, the hospital must answer within 30 days. It can take 30 more days once, if it explains why in writing.
- It can charge only a reasonable fee, based on what the copy costs to make.
- These rules also cover an electronic copy your parent asks, in writing, to have sent to you.
