How to see your parent’s MyChart account with your own login, how to get copies from the records office, and how to keep the results in one place.

See your parent’s MyChart account
Hospitals call this proxy access, or family access. You sign in with your own login and switch to your parent’s account, so you never need their password. At Montefiore: the patient invites you; for a parent with diminished capacity, you send a request.
- 1Your parent logs in to their own account and opens the Family & Caregiver Proxy Access page from the menu.
- 2They select Invite Access and complete the invitation form.
- 3Once it is approved, you switch between accounts from the top right menu.
- 4If your parent has diminished capacity, log in to your own account, open the same page, select Request Access and complete the form.
- 5Allow up to 7 business days for a request. Staff reply by MyChart message.
Get copies without the portal
From the Health Information Management Department.
- 1Download the authorization form (ROI4987) and its instructions. Your parent fills it in, signs it and names you.
- 2Mail it to Montefiore Einstein, Attention: Health Information Management Department, 111 East 210th Street, Bronx, NY 10467.
- 3If you hold your parent's power of attorney, you can use the online request form on the page instead.
- 4Questions: call 718-920-4921.
- Family members:
- The online request is for a patient, a parent of a minor child, or a caregiver with legal power of attorney. For an incapacitated patient, the representative must enter their relationship to the patient.
- Time, as the hospital states it:
- Requests are processed within ten days after the authorization is received.

Your parent’s rights
- Your parent has a legal right to copies of their own records.
- When your parent asks, the hospital must answer within 30 days. It can take 30 more days once, if it explains why in writing.
- It can charge only a reasonable fee, based on what the copy costs to make.
- These rules also cover an electronic copy your parent asks, in writing, to have sent to you.

How do I keep my parent's Montefiore results in one place?
