See your parent’s Main Line Health MyChart account
Hospitals call this proxy access, or family access. You sign in with your own login and switch to your parent’s account, so you never need their password. At Main Line: a registered user requests it with a form, for a patient who wishes to grant access.
- 1Download and complete the form Authorization for Proxy Access for an Adult Patient.
- 2Follow the form's instructions to send it. The page does not say where it goes.
- 3Once access is granted, log in to MyChart. You will see an icon for each profile you can open.
Get copies without the portal
From the Health Information Management.
- 1Download the Authorization for Disclosure of Health Information form. Read the instructions on page 2.
- 2Your parent fills in the records wanted, the purpose, your name and address, an end date, and signs it.
- 3Send it by mail, email or fax (610.356.3167), as the page lists.
- 4Psychiatric, drug or alcohol, HIV/AIDS and sexual abuse records need the form to name them.
- Family members:
- Records can be released to anyone the patient authorized in writing. Authorizations signed by a representative must include a copy of the guardianship papers or power of attorney.
- Cost, as the hospital states it:
- In accordance with PA state law, fees are charged for copies: $2.00 per page for pages 1-20, $1.48 for pages 21-60, $0.52 for pages 61-end, plus postage if mailed. No charge for records provided directly to your physician.

Your parent’s rights
- Your parent has a legal right to copies of their own records.
- When your parent asks, the hospital must answer within 30 days. It can take 30 more days once, if it explains why in writing.
- It can charge only a reasonable fee, based on what the copy costs to make.
- These rules also cover an electronic copy your parent asks, in writing, to have sent to you.
Keep the results in one place
Vita Health Coach is an iPhone app that keeps each family member's results in their own file and explains each new result in plain words. Once you have access, add your parent, tap Connect a hospital and search for Main Line. Paper copies work too: take a photo of each page.
