See your parent’s MyHealth account
Hospitals call this proxy access, or family access. You sign in with your own login and switch to your parent’s account, so you never need their password. At Legacy Health: you and the patient both sign a form.
- 1Print Legacy's Request for MyHealth Proxy Access form.
- 2Your parent and you both fill in and sign it. Your parent chooses full or partial access.
- 3Return it to any Legacy Health registration desk or your parent's Legacy clinic.
- 4You need your own MyHealth account. Once the accounts are linked, you see your parent's name when you log in.
- 5Questions: MyHealth Customer Service, Oregon 503-415-4835 or Washington 360-487-1075.

- These rules also cover an electronic copy your parent asks, in writing, to have sent to you.
Keep the results in one place
Vita Health Coach is an iPhone app that keeps each family member's results in their own file and explains each new result in plain words. Once you have access, add your parent, tap Connect a hospital and search for Legacy Health. Paper copies work too: take a photo of each page.
