See your parent’s MyJeffersonHealth account
Hospitals call this proxy access, or family access. You sign in with your own login and switch to your parent’s account, so you never need their password. At Jefferson: you and your parent hand in proxy forms together.
- 1Your parent can give a spouse or an adult over 18 who helps manage their health full access. Jefferson calls this Adult Proxy access.
- 2Ask your parent's provider for the required forms and documentation.
- 3You and your parent bring the completed forms to the physician's office or a Form Drop-off Location. Both of you must be there and show photo ID, such as a driver's license or US passport.

Your parent’s rights
- Your parent has a legal right to copies of their own records.
- When your parent asks, the hospital must answer within 30 days. It can take 30 more days once, if it explains why in writing.
- It can charge only a reasonable fee, based on what the copy costs to make.
- These rules also cover an electronic copy your parent asks, in writing, to have sent to you.
Keep the results in one place
Vita Health Coach is an iPhone app that keeps each family member's results in their own file and explains each new result in plain words. Once you have access, add your parent, tap Connect a hospital and search for Jefferson. Paper copies work too: take a photo of each page.
