See your parent’s MyChart (Intermountain Health app) account
Hospitals call this proxy access, or family access. You sign in with your own login and switch to your parent’s account, so you never need their password. At Intermountain: the patient sends the invitation from their own account.
- 1Your parent opens the menu, scrolls to Sharing and taps Sharing Hub.
- 2They tap Family member, close friend, or caretaker and follow the prompts to send an invitation.
- 3You need your own MyChart account, and you log in to it to reach your parent's information.
- 4The FAQ also mentions a Proxy Application Form, so ask if it applies to you.
- 5If you hold legal authority, such as guardianship, records can be requested through the Health Information department.
Get copies without the portal
From the Centralized Release of Information / Medical Records.
- 1Download the authorization form for sending records to someone else.
- 2Your parent fills it in, signs it, and names you to receive the records. Choose the format: paper, CD, USB or email.
- 3Send it to the office for the state where your parent was treated. Colorado, Nevada, Montana, Wyoming: email, fax or mail to Broomfield, CO. Utah, Idaho: email, fax or mail to Murray, UT. Details are on the form.
- Family members:
- The form has a signature line for a guardian or personal representative, with their relationship to the patient.
- Cost, as the hospital states it:
- I may review my medical record without a charge. If I request copies of my medical record, I may be charged a fee. (from the form)

Your parent’s rights
- Your parent has a legal right to copies of their own records.
- When your parent asks, the hospital must answer within 30 days. It can take 30 more days once, if it explains why in writing.
- It can charge only a reasonable fee, based on what the copy costs to make.
- These rules also cover an electronic copy your parent asks, in writing, to have sent to you.
Keep the results in one place
Vita Health Coach is an iPhone app that keeps each family member's results in their own file and explains each new result in plain words. Once you have access, add your parent, tap Connect a hospital and search for Intermountain. Paper copies work too: take a photo of each page.
