How to see your parent’s MyChart account with your own login, how to get copies from the records office, and how to keep the results in one place.

See your parent’s MyChart account
Hospitals call this proxy access, or family access. You sign in with your own login and switch to your parent’s account, so you never need their password. At Essentia: the patient sends an invitation from their own account or signs a form at a clinic.
- 1Your parent logs in to MyChart, selects Sharing Hub from the main menu and follows the prompts to send you an invitation. This is the quickest way.
- 2Or your parent asks at any Essentia clinic or hospital for the proxy authorization form. Access starts once the form is completed and processed.
- 3You log in to your own MyChart account to see your parent's information. If you are not an Essentia patient, you fill in and send a form to create your account.
- 4Your parent can end your access at any time in MyChart or by writing to Essentia.
Get copies without the portal
From the Release of Information (ROI) Office, Essentia Health HIM.
- 1Download the Authorization for Use and Disclosure of Protected Health Information form, or use the online request tool on the page.
- 2Your parent fills it in, signs it and names you to receive the records.
- 3Fax it to 920-593-3114 or mail it to Essentia Health-HIM, 502 East Second Street, Duluth MN 55805.
- 4For questions or status, call the records help line at 866-203-7454.
- Family members:
- A separate Verbal Authorization Form lets Essentia talk about medical and billing information with a person your parent names. Your parent must sign it by hand. An authorization is valid for one year unless the form says otherwise.
- Time, as the hospital states it:
- "Usually available within 2-3 business days"

- Your parent has a legal right to copies of their own records.
- When your parent asks, the hospital must answer within 30 days. It can take 30 more days once, if it explains why in writing.
- It can charge only a reasonable fee, based on what the copy costs to make.
- These rules also cover an electronic copy your parent asks, in writing, to have sent to you.

How do I keep my parent's Essentia results in one place?
