How to see your parent’s MyEndeavor account with your own login, how to get copies from the records office, and how to keep the results in one place.
From Endeavor Health’s own pages, checked on November 3, 2026. Steps can change, so follow the official links.
See your parent’s MyEndeavor account
Hospitals call this proxy access, or family access. You sign in with your own login and switch to your parent’s account, so you never need their password. At Endeavor: the patient sends an invitation, or you request access with a form both of you sign.
- 1Edward-Elmhurst and NCH MyChart patients move to MyEndeavor on November 7, 2026. Do not send forms before that date.
- 2Invite route: your parent logs in, opens Sharing Hub, selects Manage friends and family access, then Invite friends or family.
- 3Form route: complete the Adult Proxy Access Form. You and your parent both sign it.
- 4To send it online: Message My Care Team, Ask a customer service question, Submit proxy form, then attach the form and send.
- 5If your parent lacks capacity, a legal guardian or health care power of attorney completes the form. Extra legal papers are required.
Get copies without the portal
From the Health Information Management.
- 1On the page, choose the area where your parent was treated. Each area has its own form and contacts. The form linked here is for the northern suburbs and Chicago north side.
- 2Your parent fills in the Authorization to Disclose Records form, signs it, and names you to receive the records.
- 3Send it by mail, email (releaseforms@endeavorhealth.org) or fax, or use the online request form, as the page lists for that area.
- 4To pick up in person, the patient must sign the form and write your name as the recipient. Bring photo ID.
- Family members:
- If someone is picking up records for the patient, the form must be completed and signed by the patient, with the recipient's name listed.
- Time, as the hospital states it:
- Turnaround time: 7–10 business days (in-person requests, northern suburbs and Chicago north side).

Your parent’s rights
- Your parent has a legal right to copies of their own records.
- When your parent asks, the hospital must answer within 30 days. It can take 30 more days once, if it explains why in writing.
- It can charge only a reasonable fee, based on what the copy costs to make.
- These rules also cover an electronic copy your parent asks, in writing, to have sent to you.
Keep the results in one place
Vita Health Coach is an iPhone app that keeps each family member's results in their own file and explains each new result in plain words. Once you have access, add your parent, tap Connect a hospital and search for Endeavor. Paper copies work too: take a photo of each page.
