See your parent’s My Duke Health account
Hospitals call this proxy access, or family access. You sign in with your own login and switch to your parent’s account, so you never need their password. At Duke: the patient requests it with an authorization form.
- 1Your parent completes the Authorization for Adult Proxy Access to My Duke Health form, found on Duke's medical records page.
- 2They bring it to the next provider visit, or fax it to Health Information Management using the number on the form.
- 3Access is at Duke's discretion, and you must agree to the terms and conditions.
- 4Duke's FAQ also lists a portal route: Menu, Sharing, Sharing Hub, then the family, close friend or caretaker option.
- 5If your parent is incapacitated, an adult with legal decision-making authority asks through the physician, with legal papers and a separate form.
Get copies without the portal
From the Health Information Management – Release of Information.
- 1Download the Duke Health authorization form. A Spanish form and instructions are on the forms page.
- 2Your parent fills it in, signs it, and names you to receive the records.
- 3Send it by fax (919-620-5165), email (ROI-Requestor3@dm.duke.edu) or mail to Release of Information, as the page lists.
- Family members:
- Written authorization is required. Duke also has a separate form so a friend or family member can be given information by phone.

Your parent’s rights
- Your parent has a legal right to copies of their own records.
- When your parent asks, the hospital must answer within 30 days. It can take 30 more days once, if it explains why in writing.
- It can charge only a reasonable fee, based on what the copy costs to make.
- These rules also cover an electronic copy your parent asks, in writing, to have sent to you.
Keep the results in one place
Vita Health Coach is an iPhone app that keeps each family member's results in their own file and explains each new result in plain words. Once you have access, add your parent, tap Connect a hospital and search for Duke. Paper copies work too: take a photo of each page.
