See your parent’s Allina Health account account
Hospitals call this proxy access, or family access. You sign in with your own login and switch to your parent’s account, so you never need their password. At Allina: you and your parent sign a paper adult proxy form.
- 1Download the Adult Proxy Form. You fill in the proxy part and your parent fills in the patient part. Both parts ask for the last 4 digits of the SSN and date of birth.
- 2You and your parent both sign and date the form. You will see your parent's record through your own MyChart account.
- 3Mail it to 49000 Allina Health Customer Experience Center, 8880 Evergreen Blvd, Coon Rapids, MN 55433, or fax it to 612-262-1424. Do not deliver it in person.
- 4Access lasts five years. Minnesota law requires a new form every five years to renew it.
- 5Once access is set up, you sign in, open the menu at the top right and click your parent's name.
Get copies without the portal
From the Allina Health Health Information/ROI.
- 1Your parent can send the request online from their Allina Health account, using Request to release and disclose patient information.
- 2Or download the Authorization to Release and Disclose Patient Information form. Your parent fills it in, names you under Send My Medical Records To, and signs and dates it.
- 3Mail it to Allina Health, Attn: Health Information/ROI, Mail Route 10203, PO Box 43, Minneapolis, MN 55440-0043, or fax it to 612-262-2323. Questions: 612-262-2300.
- Family members:
- A legal guardian may sign the form instead and must attach the document showing authority to act for the patient.
- Cost, as the hospital states it:
- Fees may be charged in accordance with MN Statute 144.2923 and Federal Rule 45 C.F.R. §164.524
- Time, as the hospital states it:
- Please allow 7-10 days for all requests to be processed and sent to the recipient

Your parent’s rights
- Your parent has a legal right to copies of their own records.
- When your parent asks, the hospital must answer within 30 days. It can take 30 more days once, if it explains why in writing.
- It can charge only a reasonable fee, based on what the copy costs to make.
- These rules also cover an electronic copy your parent asks, in writing, to have sent to you.
Keep the results in one place
Vita Health Coach is an iPhone app that keeps each family member's results in their own file and explains each new result in plain words. Once you have access, add your parent, tap Connect a hospital and search for Allina. Paper copies work too: take a photo of each page.
