See your parent’s AdventHealth account (MyChart) account
Hospitals call this proxy access, or family access. You sign in with your own login and switch to your parent’s account, so you never need their password. At AdventHealth: the patient sends a proxy invite from their own account.
- 1Your parent logs in to their AdventHealth account and sends you a proxy invite from the Friends and Family Access page.
- 2You need your own AdventHealth account and must be at least 18. Spouses cannot share one account.
- 3If you are with your parent when they register at an AdventHealth hospital, Patient Access staff can send you an invitation.
- 4For help, call AdventHealth tech support at 855-238-8791.
Get copies without the portal
From the Health Information Management (HIM) department of each facility.
- 1Each AdventHealth hospital has its own Health Information Management office.
- 2On the page, open Online Medical Records Request (Patients) and find the hospital your parent used. Each one shows a phone number, a fax number and an Online eRequest Form.
- 3Your parent gives written authorization and names you to receive the records.
- 4Send it through that hospital's Online eRequest Form or to its fax number. Questions: call the number listed for that hospital.
- Family members:
- Paper copies of medical records may be released upon receipt of written authorization of patients over the age of 18 or a legal guardian.
- Cost, as the hospital states it:
- There might be a charge for medical records if being a request by a patient or patient representative.
- Time, as the hospital states it:
- Once authorization is received, it may take up to 10 days to process your request.
Your parent’s rights
- Your parent has a legal right to copies of their own records.
- When your parent asks, the hospital must answer within 30 days. It can take 30 more days once, if it explains why in writing.
- It can charge only a reasonable fee, based on what the copy costs to make.
- These rules also cover an electronic copy your parent asks, in writing, to have sent to you.
Keep the results in one place
Vita Health Coach is an iPhone app that keeps each family member's results in their own file and explains each new result in plain words. Once you have access, add your parent, tap Connect a hospital and search for AdventHealth. Paper copies work too: take a photo of each page.
